Comparison of Common Bile Duct Exploration and Endoscopic Sphincterotomy for CBD Stones

Abstract
Background/Method: Ninety-four patients with common bile duct stones were entered into a retro-spective study of endoscopic sphincterotomy(EST) and stone clearance versus open common bile duct exploration(CBDE) to compare the clinical effect. The CBDE group(n=50) included 14 patients whose stones were not successfully removed after EST. The 257 group(n=44) included 4 patients whose stones were found after the laparoscopic cholecystectomy.

Results: The success rate of EST was 75.9%. Mean length of hospital stay was 12.8¡¾11.1 days in CBDE group and 14.5¡¾5.8days in EST group, but the difference between the two groups was not statistically significant(P>0.05). In EST group, EST was performed 2.14¡¾0.88 times per patient and the average duration from the last EST to operation was 4.1¡¾2.3 days. There was no mortality in both groups. Three cases of retained stones and 5 complications were found in CBDE group and 4 complications in EST group.

Conclusion: The difference of mean length of hospital stay between two groups was not statistically significant(P>0.05). CBDE was a reliable method and applicable to CBD stones failed with EST in EST group, the mean length of hospital stay could be shortened.
